This form is to be used in reporting of Forged and / or Counterfeit Checks that have been passed in the city limits of Lawton. By using this form, the merchant/business will be able to provide the necessary suspect and transaction information required for prosecution purposes.  This form will only include checks that have been passed within the legal jurisdiction of the Lawton Police Department, which is the city limits of Lawton.  

Print a copy of the form, fill it out and then call police dispatch at 580-581-3272. An officer will come out, pick up the check, the completed form and make an offense report.
